Dallas, Texas — Prism Care, Inc., a leading Dallas-based care management company specializing in Remote Patient Monitoring (RPM), Chronic Care Management (CCM), and other technology-enabled preventive care services, today announced its merger with Level Healthcare Solutions, LLC. Prism Care will serve as the parent company of the combined organization.

Level Healthcare Solutions, founded in 2020 and also headquartered in the Dallas/Ft. Worth area, provides remote care services closely aligned with Prism Care’s mission and clinical model. The merger strengthens Prism Care’s nurse-led, AI-empowered care management capabilities while expanding the number of clinics and patients served nationwide. For clinics, the combined organization offers deeper clinical support, more robust operational infrastructure, and expanded revenue opportunities through high-touch preventive care programs.
