Vital Rewards Program Terms and Conditions

The Vital Rewards program (“Program”) participation and its benefits are offered at the sole discretion of Vital Rewards.  At its sole discretion, Vital Rewards has the right to modify, add or delete any of the Program rules, terms, conditions, benefits or Rewards, in whole or in part, at any time, with or without notice, even though such changes may affect the value of points already accumulated, the value of the Rewards and/or the time for redemption.

Select participants in Prism Care are eligible for enrollment in Vital Rewards.

• A point is awarded for each day that at least one vital reading is received in the Prism Care system
• A bonus point is awarded at the end of each week that the participant successfully submitted vitals for all 7 days
• For each 30 points accrued by the participant, $5 in value Amazon gift card value is achieved

Rewards are redeemable online only through the Vital Rewards redemption process.  Rewards have no cash value and cash will not be paid for unused portions of the Reward. Rewards are not transferable and Vital Rewards reserves the right to limit the time within which you may redeem Rewards.

The Program has no predetermined termination date and may continue until such time as Vital Rewards, in its sole discretion, elects to designate a Program termination date. Vital Rewards has the right to end the Program at any time without providing written notice to you.

Data gathered as part of this program is governed by the Prism Care Privacy Policy.  The Privacy Policy is available online at https://prismcare.com/privacypolicy/.

Modification of these terms and conditions may only be made in writing by an authorized representative of Vital Rewards. All interpretations of Program terms and conditions shall be at the sole discretion of Vital Rewards and such interpretations are binding on you.

You are responsible for notifying Vital Rewards of any changes of email address. Vital Rewards is not responsible for Rewards which are lost, delayed or misdirected in email or otherwise during transit. Program points and Rewards may be subject to federal, state, or other taxes and such taxes are your sole responsibility.

Participants must be at least 18 years of age and must be a United States resident. A valid email address is required for participation. Participation in the Program is void if prohibited by law in the jurisdiction in which the prospective participant resides. The Program is a United States program and is governed by United States law. If in any particular state the Program or Vital Rewards has to apply for any special license, bonding, permit or other governmental regulatory approval, the Program will be void in that particular state. The laws of the State of Texas apply to the Program and these Terms and Conditions without regard to any conflict of law rules that may require the application of the laws of another jurisdiction.
